Effective January 1, 2009

The Basic and Enhanced Superior Vision Services, Inc., plans are fully insured and voluntary. Employees may elect to enroll themselves and any eligible dependents in one of the vision plans - whether or not the employee or dependents are enrolled in the State of Kansas medical coverage. Dependent children enrolled in one of the Superior Vision Services, Inc., plans must be the same as those if enrolled in the medical plan. Enrollment cannot be changed during the plan year unless due to either a newly eligible dependent or to a dependent becoming ineligible.

Contact Lenses Online-a value added benefit available to participants in the Superior Vision Basic or Enhanced plans. Superior Vision and Arlington Lens Supply offer an online mail order contact lenses program to participants. This is an optional benefit and participants may continue to purchase their contacts from their provider if they wish. The online site www.SVContacts.com is a network provider that offers a wide variety of contact lenses. Lenses are shipped directly to the participant based on the doctor’s prescription. This is just another convenient option to participants to use for purchasing contact lenses.

When Eligible: 1st day of the month following 60 days of employment or during open enrollment period.

Who Pays: The employee contributes to this plan. There is no university contribution.
